Fluconazole Use in Elderly Patients with Impaired Renal Function

Pharmacokinetics and Drug Interactions

  • The National Comprehensive Cancer Network recommends being aware of fluconazole's significant drug interactions through inhibition of cytochrome P450 enzymes (CYP3A4, CYP2C9, CYP2C19), which may be particularly relevant in elderly patients who often take multiple medications 1
  • QTc prolongation is a potential concern when fluconazole is combined with other QTc-prolonging drugs commonly used in elderly patients (e.g., fluoroquinolones, macrolides) 1

Monitoring and Dosing Recommendations

  • The guideline society recommends calculating creatinine clearance before initiating therapy to determine appropriate dosing 2
  • Relying solely on serum creatinine without calculating creatinine clearance can lead to inappropriate dosing in elderly patients 2

Alternative Antifungal Options

  • For patients with severe renal impairment, consider alternative antifungal agents with less renal clearance when appropriate for the infection 2
  • Echinocandins may be preferred for certain Candida infections in patients with significant renal impairment 3
  • For mucocutaneous candidiasis, topical options may be considered to avoid systemic exposure 4
