Treatment Escalation for Relapsed Enterococcal Prostatitis

Primary Treatment Algorithm

  • The American Heart Association recommends combination therapy with daptomycin 10-12 mg/kg/day IV and ampicillin 2g IV every 6 hours for 8 weeks for ampicillin-susceptible enterococcal prostatitis, as it demonstrates the greatest synergistic activity compared to other combinations 1, 2
  • For ampicillin-susceptible enterococcal prostatitis, the recommended treatment is high-dose daptomycin 10-12 mg/kg/day IV combined with ampicillin 2g IV every 6 hours for 8 weeks 1, 2
  • If ampicillin-resistant or beta-lactam intolerant, linezolid 600 mg PO or IV every 12 hours for 6 weeks is recommended, with success rates of 80-86% for chronic enterococcal prostatitis 1, 2

Mandatory Monitoring Requirements

  • Weekly CPK levels throughout high-dose daptomycin therapy are necessary due to significant myopathy risk, and therapy should be discontinued immediately if CPK rises significantly or muscle pain/weakness develops 1, 2
  • Weekly complete blood counts are required if using linezolid for more than 2 weeks due to myelosuppression risk 1, 2
  • Weekly renal function tests are necessary when using multiple nephrotoxic agents 5

Expected Clinical Timeline

  • Initial symptom improvement is expected within 5-7 days, with a more complete clinical response within 10-14 days, and microbiological documentation of cure within 2-4 weeks 1, 2

Infectious Disease Consultation

  • Management of relapsed enterococcal infections should involve infectious disease consultation as standard of care (Class I recommendation) 7, 5, 1, 2
