Topiramate Dosing Guidelines

Introduction to Topiramate Dosing

  • The American Medical Association recommends a maximum dose of 15 mg phentermine/92 mg topiramate daily for obesity management 1, 2

Obesity Management

  • The National Institute of Diabetes and Digestive and Kidney Diseases suggests a mandatory titration schedule for phentermine-topiramate ER: Days 1-14: 3.75 mg/23 mg daily, Day 15 onward: 7.5 mg/46 mg daily, and evaluate at 12 weeks: if <3% weight loss, increase to 11.25 mg/69 mg daily for 14 days, then 15 mg/92 mg daily 2, 3
  • Discontinue phentermine-topiramate ER if <5% weight loss after 12 weeks at maximum dose (15/92 mg) 3, 4

Neuropathic Pain Management

  • The American Diabetes Association lists topiramate as an option at 25-100 mg/day for painful diabetic peripheral neuropathy, but not a first-line agent 5
  • First-line agents for neuropathic pain include duloxetine, pregabalin, and tricyclic antidepressants 5

Critical Monitoring and Precautions

  • The American Heart Association recommends monitoring serum bicarbonate regularly due to risk of metabolic acidosis 1, 6
  • The American College of Gastroenterology suggests tapering gradually if discontinuing topiramate to avoid increased seizure risk 6
  • The American College of Obstetricians and Gynecologists states that topiramate is contraindicated in pregnancy due to risk of cleft lip/palate; women of childbearing potential require effective contraception 2, 3
  • The American Society of Anesthesiologists recommends discontinuing phentermine-topiramate ER at least 4 days before procedures requiring general anesthesia due to risk of perioperative complications 3

Topiramate Uptitration Strategy and Monitoring

Introduction to Topiramate Uptitration

  • The American Academy of Neurology recommends liver function tests are not routinely required for topiramate monotherapy 7

Special Considerations for Topiramate Use

  • The National Kidney Foundation suggests that concurrent strong CYP3A inhibitors (itraconazole, ketoconazole, clarithromycin, protease inhibitors) may require holding or downtitrating topiramate 8
  • The Endocrine Society recommends that topiramate may increase the risk of hypoglycemia when used with insulin/sulfonylureas, requiring dose adjustments and close glucose monitoring 8
