Initial Ventilator Settings for Pressure-Controlled Mechanical Ventilation (PCMV)

Core Initial Settings

  • The American Academy of Sleep Medicine recommends setting initial tidal volume to 6-8 ml/kg predicted body weight, with a minimum recommended I:E ratio of 1:2, and respiratory rate between 20-35 breaths/minute 1, 2, 3
  • The British Journal of Anaesthesia suggests maintaining plateau pressure strictly <30 cm H₂O at all times, and never exceeding 8 ml/kg predicted body weight to prevent ventilator-induced lung injury 1, 2
  • Initial PEEP should be set at 5 cm H₂O, and individualized thereafter based on oxygenation needs and driving pressure, with a minimum PEEP of 5 cm H₂O 1, 2, 3
  • FiO₂ should be set to 0.4 (40%) initially, and titrated to the lowest concentration needed to achieve SpO₂ 88-95% 2, 3

Disease-Specific Adjustments

  • For patients with ARDS, tidal volumes should be set to 4-8 ml/kg predicted body weight, with strict plateau pressure <30 cm H₂O, and prone positioning for >12 hours/day (preferably 16 hours) for severe cases 2
  • For obstructive lung disease, tidal volumes should be set to 6-8 ml/kg predicted body weight, with respiratory frequency at 10-15 breaths/minute, and I:E ratio of 1:2 to 1:4 2, 3, 4
  • For restrictive lung disease, tidal volumes should be set to 6 ml/kg predicted body weight, with respiratory frequency at 15-25 breaths/minute, and I:E ratio of approximately 1:1 3, 4

Critical Monitoring Parameters

  • Arterial blood gas should be obtained within 1-2 hours of initiating mechanical ventilation, and dynamic compliance, driving pressure, and plateau pressure should be monitored in all mechanically ventilated patients 2, 3
  • Driving pressure is a significant determinant of lung injury and should be minimized, according to the British Journal of Anaesthesia 1

Recruitment Maneuvers

  • Recruitment maneuvers should be considered when there is evidence of atelectasis, using the lowest effective pressure and shortest effective time or fewest number of breaths, as recommended by the British Journal of Anaesthesia 1, 2, 3
