Transesophageal Echocardiography in Pediatric Restrictive Cardiomyopathy
Primary Imaging Approach
- The American College of Cardiology recommends that transthoracic echocardiography (TTE) remains the first-line imaging modality for pediatric patients with suspected or confirmed endocardial fibroelastosis (EFE), as it is non-invasive, widely available, and generally provides adequate visualization in children 1, 2
Specific Indications for TOE in EFE
- The American College of Cardiology indicates that transesophageal echocardiography (TOE) is strongly recommended when significant diagnostic information cannot be obtained by TTE in any patient with congenital or acquired heart disease, including restrictive cardiomyopathy 1, 2
- TOE is recommended for perioperative monitoring and guidance during surgical resection of EFE, particularly for assessing completeness of endocardial resection, residual valvular insufficiency, and ventricular function 1, 2
- The American Heart Association suggests that TOE can be used in cases of chest wall disruptions from previous cardiac surgery or congenital anomalies of the thoracic cage that limit TTE acoustic windows 3
Important Limitations of TOE in Pediatric EFE
- The American College of Cardiology notes that general anesthesia or deep sedation is required in pediatric patients undergoing TOE, adding procedural risk and complexity, and potential for airway compromise exists 1, 2
- The American College of Cardiology recommends that TOE should only be performed by experienced operators, especially in hemodynamically unstable patients 4
Alternative Imaging Modalities
- The American Heart Association suggests that cardiac MRI has emerged as the superior non-invasive modality for definitive diagnosis and functional assessment of EFE, and should be strongly considered before proceeding to TOE 5
- Cardiac MRI provides more accurate ventricular volume and function measurements, and is not affected by body habitus, thoracic deformities, or post-surgical changes that degrade echocardiographic image quality 5
