Management of Preterm Labor at 32 Weeks Gestation
Immediate Interventions
- The American College of Obstetricians and Gynecologists recommends administering corticosteroids for fetal lung maturation and magnesium sulfate for neuroprotection at 32 weeks gestation with preterm labor, and considering tocolytics for a 48-hour delay to allow steroid completion 1
- Administer betamethasone 12 mg intramuscularly for two doses, either 12 or 24 hours apart, to reduce respiratory distress syndrome and intraventricular hemorrhage 2
- Corticosteroids should be given between 24+0 and 34+0 weeks gestation, with optimal benefit when delivery occurs within 7 days of administration, according to the American College of Obstetricians and Gynecologists 1, 2
- A single complete course of corticosteroids is critical, and multiple courses are not recommended due to potential harm, including reduced birthweight and head circumference, as stated by the European Society of Cardiology 2, 3
Tocolytic Therapy Considerations
- Tocolytics may delay delivery for 48-72 hours, allowing time for corticosteroid administration and maternal transfer to a tertiary care facility, but do not consistently improve neonatal outcomes, according to the American College of Obstetricians and Gynecologists 1
Maternal Assessment and Monitoring
- Assess for intraamniotic infection, placental abruption, severe preeclampsia, or non-reassuring fetal status, which require immediate delivery, as recommended by the European Society of Cardiology 3, 4
- Check for preterm premature rupture of membranes and administer broad-spectrum antibiotics to prolong pregnancy and reduce neonatal infections, according to the American College of Obstetricians and Gynecologists 1
Delivery Planning and Timing
- At 32 weeks gestation, preterm neonatal survival rate is high (95%) with low risk of neurological sequelae, supporting delivery at 32 weeks if maternal or fetal conditions deteriorate, as stated by the European Society of Cardiology 6, 7
- Vaginal delivery with epidural anesthesia is preferred for most cases to minimize hemodynamic stress, according to the European Society of Cardiology 6, 7
Special Circumstances
- With fetal growth restriction, continue surveillance with serial Doppler every 2 weeks if umbilical artery Doppler is normal, and plan delivery at 34 weeks unless earlier delivery is indicated, as recommended by the American College of Obstetricians and Gynecologists 2, 5
- Control blood pressure to 110-140/80-85 mmHg using labetalol, methyldopa, or nifedipine in cases of hypertensive disorders, and monitor closely for development of preeclampsia, according to the European Society of Cardiology 2, 3
