Treatment of Urinary Tract Infection Caused by Citrobacter freundii

Antimicrobial Susceptibility and Treatment

  • Ciprofloxacin is effective against C. freundii with 80.6% susceptibility, if local resistance patterns permit, according to the European Urology guidelines 1
  • The European Urology guidelines recommend obtaining urine culture and susceptibility testing before initiating therapy 2
  • The Clinical Infectious Diseases guidelines recommend basing final treatment on culture results and susceptibility patterns 3
  • For complicated UTI with systemic symptoms, consider a combination therapy with amoxicillin plus an aminoglycoside or a second-generation cephalosporin plus an aminoglycoside, as recommended by the European Urology guidelines 2
  • For uncomplicated UTI without systemic symptoms, consider ciprofloxacin if local resistance is <10%, as recommended by the European Urology guidelines 2
  • Adjust therapy based on susceptibility results, as recommended by the Clinical Infectious Diseases guidelines 3
  • The treatment duration for uncomplicated UTI is 7 days, as recommended by the Clinical Infectious Diseases guidelines 3
  • The treatment duration for complicated UTI is 7-14 days, as recommended by the European Urology guidelines 2
  • For males where prostatitis cannot be excluded, the treatment duration is 14 days, as recommended by the European Urology guidelines 2
  • Consider underlying anatomical or functional abnormalities for patients with recurrent UTIs, as recommended by the European Urology guidelines 2
  • The Pediatrics guidelines recommend treating positive bacterial pathogens in urine with targeted antimicrobial therapy for a duration consistent with the nature of the disease, responsible organism, and response to treatment 4
  • If symptoms persist despite appropriate antimicrobial therapy, repeat urine culture to assess for ongoing bacteriuria, as recommended by The Journal of Urology guidelines 5
