Comorbidity of Anxiety Disorders and OCD

Diagnostic Considerations

  • The American Psychiatric Association recommends that anxiety disorders and OCD can co-occur as separate diagnoses, with anxiety disorders being the most common comorbid conditions with OCD, occurring in approximately 90% of individuals with lifetime OCD 1
  • The DSM-5 diagnostic criteria for OCD explicitly state that the diagnosis should only be excluded if "the disturbance is not better explained by the symptoms of another mental disorder," allowing for multiple diagnoses 2
  • A patient can have both OCD and an anxiety disorder when the obsessions and compulsions are distinct from the anxiety symptoms, and each condition causes independent functional impairment 2

Treatment Implications

  • The American College of Psychiatrists suggests that SSRIs are first-line pharmacotherapy for both OCD and most anxiety disorders, making them particularly useful in comorbid presentations 4
  • Cognitive Behavioral Therapy with Exposure and Response Prevention (ERP) remains the gold standard for OCD, even in the presence of comorbid anxiety disorders 4
  • Addressing comorbid conditions in treatment planning is crucial, as the presence of comorbid anxiety disorders may require integrated treatment approaches 4

Clinical Assessment Approach

  • A systematic assessment of whether obsessions are truly intrusive and ego-dystonic, and whether compulsions are present as repetitive behaviors or mental acts aimed at reducing anxiety from obsessions, is necessary to determine if both diagnoses apply 2
  • Evaluating whether anxiety symptoms exist independently of OCD symptoms and cause separate functional impairment is also essential 1
  • Assessing the temporal relationship between symptom onset can help determine if anxiety symptoms preceded OCD or emerged simultaneously 1
