Diagnostic Criteria and Investigations for POEMS Syndrome

Introduction to POEMS Syndrome

  • The best investigation for POEMS syndrome includes serum vascular endothelial growth factor (VEGF) levels, along with bone marrow sampling, and imaging studies (CT scan, X-rays, and FDG-PET) to evaluate the presence of major and minor diagnostic criteria 1, 2

Diagnostic Criteria

  • POEMS syndrome is diagnosed based on the presence of major and minor criteria, including polyneuropathy, monoclonal plasma cell disorder, elevated vascular endothelial growth factor (VEGF) levels, sclerotic bone lesions, and Castleman disease 1, 2
  • The major criteria for POEMS syndrome include monoclonal plasma cell disorder, which is almost always λ-restricted 1, 2
  • Sclerotic bone lesions are a major criterion for POEMS syndrome diagnosis 1, 2
  • Castleman disease is a major criterion for POEMS syndrome diagnosis 1, 2
  • Organomegaly, extravascular volume overload, endocrinopathy, skin changes, papilledema, and thrombocytosis/polycythemia are minor criteria for POEMS syndrome diagnosis 1, 2

Essential Diagnostic Workup

  • Serum protein electrophoresis with immunofixation is a crucial laboratory test for diagnosing POEMS syndrome 1, 2, 3
  • Urine protein electrophoresis with immunofixation (24-hour collection) is necessary for diagnosing POEMS syndrome 1, 2, 3
  • Serum free light chain assay is a useful test for diagnosing POEMS syndrome 1, 2
  • Complete blood count with differential is necessary to assess for thrombocytosis in POEMS syndrome patients 3
  • Comprehensive metabolic panel, including calcium and creatinine, is essential for POEMS syndrome diagnosis 3
  • Serum VEGF levels are a crucial diagnostic marker for POEMS syndrome 1, 2
  • Endocrine function tests, such as thyroid, adrenal, and gonadal function, are necessary for POEMS syndrome diagnosis 1, 2

Bone Marrow Assessment and Imaging Studies

  • Bone marrow aspiration and biopsy are essential for diagnosing POEMS syndrome 1, 2, 3
  • Flow cytometry and immunohistochemistry are necessary to detect clonal plasma cells in POEMS syndrome patients 3
  • Cytogenetic analysis by FISH is a useful test for POEMS syndrome diagnosis 3
  • Whole body X-rays are necessary to detect sclerotic bone lesions in POEMS syndrome patients 1, 2
  • CT scan of the abdomen and pelvis is essential to assess organomegaly in POEMS syndrome patients 1, 2
  • FDG-PET scan is a useful imaging study to identify bone lesions and assess disease activity in POEMS syndrome patients 1, 2
  • MRI is necessary if spinal cord compression or plasmacytomas are suspected in POEMS syndrome patients 3

Additional Tests

  • Pulmonary function tests are necessary for POEMS syndrome patients 1, 2
  • Echocardiogram is essential to assess cardiac function in POEMS syndrome patients 1, 2
