Insulin Glargine Dispensing Guidelines

Introduction to Insulin Glargine Therapy

  • For a patient weighing 95.98 kg, the American College of Physicians recommends dispensing 3 Lantus (insulin glargine) pens to provide adequate coverage for a month's supply based on the recommended starting dose of 0.2-0.3 units/kg/day 1

Dose Calculation and Titration

  • The recommended starting dose for basal insulin therapy in type 2 diabetes is 0.1-0.2 units/kg/day, according to the American College of Physicians 1
  • For a 95.98 kg patient, the calculated daily dose is 19.2 units at 0.2 units/kg/day and 28.8 units at 0.3 units/kg/day 1
  • The dose should be titrated by 2-4 units every 3-4 days until fasting blood glucose reaches target range (80-130 mg/dL), as recommended by the American College of Physicians 1
  • If fasting glucose is ≥180 mg/dL, the dose should be increased by 4 units 1

Administration and Monitoring

  • Ensure proper injection technique and rotation of injection sites to maintain consistent absorption, as advised by the American Diabetes Association 2
  • When used with metformin and tirzepatide, monitor for potential hypoglycemia as the tirzepatide effect increases over weeks, according to the American College of Physicians 1
