Neutropenia Evaluation and Management

Definition and Severity

  • Neutropenia is defined as ANC <1,500 cells/mm³, with severe neutropenia at <500 cells/mm³, according to the National Comprehensive Cancer Network and Clinical Infectious Diseases guidelines 1, 2

Clinical Approach

  • Severe neutropenia (ANC <500 cells/mm³) with fever constitutes a medical emergency requiring immediate broad-spectrum antibiotics, as recommended by the National Comprehensive Cancer Network and Clinical Infectious Diseases guidelines 1, 2
  • Fever is defined as single oral temperature ≥38.3°C (101°F) or ≥38.0°C (100.4°F) for 1 hour, according to Clinical Infectious Diseases guidelines 2
  • Empiric antibiotics should include antipseudomonal coverage (cefepime, ceftazidime, or carbapenem), as recommended by Clinical Infectious Diseases guidelines 2

Treatment

  • Consider filgrastim 5 mcg/kg/day subcutaneously if high-risk features present (sepsis, pneumonia, anticipated prolonged neutropenia), although this recommendation is from Praxis Medical Insights, it is not the preferred source, however, no other source is available for this specific fact 3, 4
