Management of Breakthrough Psychotic Symptoms on Aripiprazole

Monitoring and Adjunctive Considerations

  • The American Academy of Family Physicians recommends monitoring for extrapyramidal symptoms (EPS) including akathisia, as well as headache, agitation, anxiety, and insomnia, which are common adverse effects at therapeutic doses of aripiprazole 1
  • The American Academy of Family Physicians advises against adding a benzodiazepine for psychotic symptoms, as they can paradoxically worsen confusion in some patients, and instead recommends considering alternative treatments for agitation and delirium management 2
  • The American Academy of Family Physicians suggests assessing for drug interactions, such as CYP3A4 inducers (e.g. carbamazepine, rifampin) that can lower aripiprazole levels, and considering dose adjustments for CYP2D6 poor metabolizers 1

Alternative Treatment Options

  • The American Academy of Family Physicians recommends considering alternative atypical antipsychotics, such as olanzapine 5-10mg daily or risperidone 2-4mg daily, if psychotic symptoms persist at 30mg of aripiprazole, although these alternatives carry higher risks of metabolic effects or EPS 1
  • The American Academy of Family Physicians suggests quetiapine 100-400mg daily as another option with lower EPS risk but more sedation 1
