Immediate Surgical Referral for Femoral Neck Fracture
Critical Understanding and Management
- The American Academy of Orthopaedic Surgeons recommends urgent orthopedic surgical consultation and admission for operative management within 24-48 hours for patients with femoral neck fractures, regardless of their ability to ambulate 1, 2, 3
- The presence of redness in a patient with a femoral neck fracture suggests possible skin compromise, infection risk, or inflammatory response that requires immediate evaluation and may influence surgical timing 3
- A patient's ability to walk does not exclude the possibility of a serious fracture, as those with basicervical and minimally displaced femoral neck fractures can maintain weight-bearing ability and function initially 4
Immediate Management Steps
- The American Academy of Orthopaedic Surgeons recommends implementing interdisciplinary care with orthogeriatric comanagement to decrease complications and improve outcomes in patients with femoral neck fractures (strong strength recommendation) 1, 2
- Obtain complete blood count and basic metabolic panel to assess for anemia, leucocytosis, and electrolyte disturbances in patients with femoral neck fractures 3
- Evaluate patients with femoral neck fractures for malnutrition, volume status, and cardiac/pulmonary diseases 3
Surgical Treatment Decision
- The American Academy of Orthopaedic Surgeons recommends arthroplasty as the treatment of choice for displaced femoral neck fractures in elderly patients (strong strength recommendation) 1, 2
- Hemiarthroplasty is appropriate for most elderly patients with displaced fractures 1
- Total hip arthroplasty may be considered if the patient is active and has pre-existing hip arthritis 1
- The use of cemented femoral stem is recommended (strong strength recommendation) 1, 2
- Administer tranexamic acid at the start of surgery to reduce blood loss and transfusion need (strong strength recommendation) 1, 2
Postoperative Management
- Allow immediate weight-bearing as tolerated after surgery (limited strength option) 1, 2, 5
- Continue VTE prophylaxis for 4 weeks postoperatively with enoxaparin or equivalent (strong strength recommendation) 3, 5
- Refer patients to Fracture Liaison Service or Bone Health Clinic for osteoporosis evaluation and secondary fracture prevention (strong strength recommendation) 3, 5
