Treatment of DVT with Apixaban

Primary Recommendation

  • The American College of Chest Physicians provides a strong recommendation that apixaban should be used over vitamin K antagonists for treatment-phase therapy in patients with DVT, based on moderate-certainty evidence 1, 3

Why Apixaban Over Enoxaparin

  • The American College of Chest Physicians recommends apixaban as monotherapy for patients with acute DVT, providing equivalent efficacy with superior safety and eliminating the need for parenteral therapy 1, 2

Clinical Algorithm for DVT Treatment Selection

  • For standard DVT treatment, apixaban 10 mg BID for 7 days, then 5 mg BID, is recommended as first-line therapy, with a minimum treatment duration of 3 months 1, 2
  • Apixaban remains the preferred treatment for cancer-associated DVT, with a strong recommendation and moderate-certainty evidence 1

Special Populations Requiring Caution

  • The American College of Chest Physicians recommends avoiding apixaban in patients with severe renal impairment (CrCl <15 mL/min) and considering alternative anticoagulation 4
  • For patients with moderate renal impairment, apixaban has approximately 27% renal elimination, requiring dose consideration but generally safe 4

When Enoxaparin Might Be Considered

  • Enoxaparin may be considered in patients with severe renal impairment where apixaban is contraindicated, or in patients with nausea/mucositis who cannot tolerate oral medications 1, 4
