Switching from Escitalopram to Duloxetine

Introduction to Switching Protocol

  • The Mayo Clinic recommends a cross-taper approach rather than abrupt switching to minimize withdrawal symptoms while maintaining therapeutic coverage 1

Cross-Tapering Protocol

  • Start duloxetine 30mg once daily while simultaneously reducing escitalopram from 20mg to 10mg, then discontinue escitalopram after one week while increasing duloxetine to 60mg daily 1
  • Reduce escitalopram from 20mg to 10mg daily during the first week of cross-tapering 1
  • Discontinue escitalopram completely after one week of cross-tapering and increase duloxetine to 60mg once daily 1

Monitoring and Management

  • Nausea is the most common side effect when initiating duloxetine 1
  • Monitor blood pressure and pulse regularly during the transition 1
  • Duloxetine can increase both blood pressure and pulse 1

Special Considerations

  • Gradual tapering over 10-14 days limits withdrawal symptoms from escitalopram 2
  • Consider starting duloxetine at 20mg with a slower cross-taper schedule for elderly or frail patients 1
  • Duloxetine dosage may require adjustment in patients with kidney disease 1
  • Avoid duloxetine in severe renal impairment 1

Target Dosing

  • Standard therapeutic dose: duloxetine 60mg once daily 1
  • Maximum dose: 120mg daily if clinically indicated after adequate trial at 60mg 3
