Initial Treatment for Sudden Sensorineural Hearing Loss

Systemic Corticosteroid Protocol

  • The American Academy of Otolaryngology-Head and Neck Surgery recommends initiating systemic corticosteroids immediately, ideally within 14 days of symptom onset, using prednisone 1 mg/kg/day (maximum 60 mg/day) for 7-14 days followed by a taper, as this represents the standard first-line therapy for sudden sensorineural hearing loss 1, 2
  • The primary treatment regimen is prednisone: 1 mg/kg/day (usual maximum 60 mg/day) 1, 2
  • Alternative options are methylprednisolone 48 mg/day or dexamethasone 10 mg/day 1
  • The duration of treatment is a full dose for 7-14 days, then taper over a similar time period 1, 2
  • Critical timing is treatment should begin immediately, ideally within the first 14 days of symptom onset, though benefit has been reported up to 6 weeks 1, 2
  • Do not divide doses—give as a single daily dose 1

Patient Selection Considerations

  • The American Academy of Otolaryngology-Head and Neck Surgery recommends that patients with contraindications to systemic steroids, such as insulin-dependent or poorly controlled diabetes, labile hypertension, glaucoma, active tuberculosis, peptic ulcer disease, or prior psychiatric reactions to corticosteroids, should be offered intratympanic steroid therapy as initial treatment 1, 2

Intratympanic Steroids as Initial Therapy

  • When systemic steroids are contraindicated, intratympanic administration provides equivalent outcomes: dexamethasone 24 mg/mL (compounded) or 10 mg/mL (stock) 1, 2
  • Methylprednisolone 40 mg/mL or 30 mg/mL can be used as an alternative 1, 2
  • The technique involves injecting 0.4-0.8 mL into the middle ear space, with the patient maintaining their head in the otologic position (affected ear up) for 15-30 minutes 1, 2
  • The frequency of injections is up to 4 injections over a 2-week period 1, 2

Monitoring and Follow-Up

  • Essential audiometric assessments include baseline audiometry within 14 days of symptom onset to confirm diagnosis 3, 4
  • Mid-treatment interval audiograms can be considered between intratympanic injections to guide early termination if hearing resolves 1, 2
  • Post-treatment audiogram at completion of treatment course 1, 2, 4
  • Final assessment: repeat audiometry at 6 months post-treatment regardless of perceived recovery 4
  • Monitor for adverse effects, such as hyperglycemia and hypertension in susceptible patients 1, 2

Salvage Therapy Planning

  • If incomplete recovery at 2-6 weeks after initial treatment, offer intratympanic steroid therapy as salvage treatment 3, 5
  • Typical salvage protocol: 4-6 injections of dexamethasone 5 mg/mL over 2 weeks 5, 4
  • Hyperbaric oxygen therapy combined with steroids may be considered within 1 month of onset 6, 4

Shared Decision-Making

  • The evidence supporting systemic steroids is equivocal, and potential adverse effects exist, making shared decision-making essential 1, 2
  • Educate patients about the natural history of sudden sensorineural hearing loss, benefits and risks of corticosteroid intervention, limitations of existing evidence regarding efficacy, and alternative treatment options if systemic steroids are contraindicated 1, 2, 3
