Platelet Transfusion Guidelines for Severe Dengue

Indications for Platelet Transfusion

  • The American Society of Anesthesiologists recommends transfusing platelets to maintain a count ≥50,000/μL in patients with active bleeding, and >100,000/μL for severe bleeding or invasive procedures 1, 2
  • For patients requiring invasive procedures, the Anesthesiology society recommends maintaining platelet count >50,000/μL for major surgery, and >100,000/μL for neurosurgery 3, 4, 5

Risk Stratification Approach

  • The Critical Care society considers active bleeding with platelet count <50,000/μL, need for emergency surgery, and severe bleeding with platelet count <100,000/μL as high-risk situations for platelet transfusion 1, 2, 3

Monitoring and Management Algorithm

  • The Journal of Clinical Oncology recommends monitoring for warning signs of severe dengue, including rapid fall in platelet count, high fever, and development of coagulopathy 6
  • The Anaesthesia society recommends maintaining platelet count >20,000/μL for central venous catheter insertion 4, 5

Common Pitfalls to Avoid

  • The Journal of Clinical Oncology warns against transfusing platelets based solely on platelet count without considering clinical bleeding status, and notes that serious hemorrhage in dengue can occur even at relatively normal platelet counts due to other factors affecting coagulation 7
