AAO‑HNS Guideline Recommendations for Peritonsillar Abscess Management
Acute Drainage and Antibiotic Therapy
- The American Academy of Otolaryngology–Head and Neck Surgery (AAO‑HNS) recommends that acute peritonsillar abscess be treated with incision‑and‑drainage (I&D) or needle aspiration plus antibiotics active against Group A Streptococcus and oral anaerobes; definitive tonsillectomy should be reserved for patients who have experienced more than one prior peritonsillar abscess or multiple failed drainage attempts. 1
Indications for Interval Tonsillectomy
- AAO‑HNS guidelines identify a history of > 1 peritonsillar abscess as a key modifying factor that favors performing an interval (elective) tonsillectomy after the acute episode has resolved. 1, 2
Timing of Elective Tonsillectomy
- When interval tonsillectomy is indicated, AAO‑HNS advises that it be scheduled after complete resolution of the acute infection, typically 4–6 weeks later. 2
Tonsillectomy Management in Peritonsillar Abscess
Indications for Interval (Elective) Tonsillectomy
- Patients who have experienced more than one prior peritonsillar abscess should be considered for elective tonsillectomy after the acute infection has fully resolved (typically 4–6 weeks later) 3.
Peri‑operative Antibiotic Use for Elective Tonsillectomy
- Routine peri‑operative antibiotics are not recommended for elective tonsillectomy performed after resolution of a peritonsillar abscess 3.
