Rate Control with Diltiazem in Atrial Fibrillation

Introduction to Diltiazem Administration

  • A Cardizem (diltiazem) drip can typically be administered for up to 24 hours, with the total dose over 24 hours not exceeding 2.2 grams, after which transition to oral therapy should be considered if continued rate control is needed, as recommended by the American Heart Association 1

Diltiazem Dosing and Administration

  • Intravenous diltiazem is commonly administered in two phases, starting with an initial bolus dose of 0.25 mg/kg (15-20 mg) IV over 2 minutes, which may be repeated with 0.35 mg/kg after 15 minutes if needed, according to the American College of Cardiology 2, 3
  • The standard infusion rate is 5-15 mg/hour, which can be maintained for 6-24 hours, as suggested by the American Heart Association 1, 2, 3

Contraindications and Monitoring

  • Diltiazem infusion should be avoided in patients with decompensated heart failure, as stated by the American College of Cardiology and the American Heart Association 2, 3
  • Patients should be monitored for hypotension, bradycardia, and heart failure exacerbation during diltiazem infusion, with continuous cardiac monitoring being essential, as recommended by the American Heart Association 2, 3

Transition to Oral Therapy

  • After achieving stable rate control with IV diltiazem, transition to oral therapy is recommended, with oral diltiazem being effective in maintaining rate control, although the specific effectiveness rate is not cited in the provided references 1
