Treatment of PCOS-Related Amenorrhea
Introduction to PCOS Management
- The American College of Obstetricians and Gynecologists recommends lifestyle modification targeting 5-10% weight loss through a 500-750 kcal/day deficit as the foundation of PCOS treatment, with clomiphene citrate 50 mg daily for 5 days as first-line ovulation induction if pregnancy is desired 1
Treatment Algorithm Based on Pregnancy Intention
- For patients desiring pregnancy, the Endocrine Society suggests initiating clomiphene citrate at 50 mg daily for 5 days, with approximately 80% of PCOS patients ovulating and 50% of those who ovulate conceiving 1
- Letrozole demonstrates superior live birth rates and pregnancy rates compared to clomiphene citrate and is increasingly preferred for ovulation induction 1
- The American Society for Reproductive Medicine recommends adding metformin (500-2000 mg daily) to improve ovulation frequency as an adjunct to lifestyle modification and ovulation induction agents, not as primary fertility treatment 1
Hormonal Management
- Combined oral contraceptive pills (OCPs) are optimal first-line treatment for patients not desiring pregnancy, addressing menstrual irregularity, providing endometrial protection, and suppressing ovarian androgen secretion 2
- OCPs increase sex hormone-binding globulin, regulate menstrual cycles, prevent endometrial hyperplasia, and reduce hirsutism and acne 2
Critical Metabolic Screening
- The American Diabetes Association recommends screening for type 2 diabetes with fasting glucose followed by 2-hour glucose tolerance test in all PCOS patients 2
- Obtain fasting lipid profile and calculate BMI and waist-hip ratio to assess metabolic risk 1, 2
Special Diagnostic Considerations
- Assess for typical Functional Hypothalamic Amenorrhea (FHA) causes, such as excessive exercise, underweight, caloric deficiency, and/or stress, to distinguish PCOS from FHA 3
- Low LH levels and LH to FSH ratio <1 suggest FHA rather than PCOS, which often has an LH to FSH ratio >2 3
Common Pitfalls to Avoid
- Do not use spironolactone in patients attempting to conceive due to contraindication in pregnancy 1, 2
- Do not exceed clomiphene dosage and duration recommendations as this worsens endometrial receptivity 1
- Do not delay lifestyle intervention, as it must be the foundation of treatment, not an afterthought 1, 2
