Catastrophic Antiphospholipid Syndrome Management

Treatment Approach

  • The American College of Physicians recommends therapeutic anticoagulation must be initiated immediately in patients with catastrophic antiphospholipid syndrome, despite extensive thrombosis, as this is a cornerstone of CAPS treatment 1, 2, 3
  • The presence of lupus anticoagulant is an indication for anticoagulation, according to the American Heart Association 4
  • Plasma exchange has been associated with improved patient survival in retrospective studies of catastrophic APS, as reported by the Kidney International journal 1, 2, 3
  • High-dose glucocorticoids (methylprednisolone 500-1000 mg daily for 3 days) are appropriate for CAPS, as recommended by the American College of Rheumatology 1, 2, 3
  • IVIG at 1 g/kg daily for 2 days or 0.4 g/kg daily for 5 days is an appropriate component of CAPS treatment, according to the American Academy of Allergy, Asthma, and Immunology 1

Anticoagulation Management

  • The American College of Cardiology recommends starting therapeutic anticoagulation immediately with unfractionated heparin or LMWH, and transitioning to warfarin (target INR 2.0-3.0) once the patient stabilizes 1, 2, 3
  • Warfarin is the anticoagulant of choice for long-term management, as recommended by the American College of Chest Physicians 5
  • Direct oral anticoagulants are contraindicated in antiphospholipid syndrome, particularly with positive lupus anticoagulant, according to the American Heart Association 5, 6

Plasma Exchange and Additional Therapies

  • Plasma exchange should be initiated early in the disease course, particularly given the multi-organ involvement, as recommended by the American Society for Apheresis 1, 2
  • Rituximab may be considered if the patient fails to respond to initial therapy or has recurrent thrombotic events despite treatment, according to the American College of Rheumatology 1
  • Emerging evidence supports eculizumab (complement inhibitor) for refractory catastrophic APS, as reported by the Kidney International journal 1, 3
