Management of Palpitations in Adolescents on Stimulants

Cardiovascular Assessment

  • Baseline vital signs, including blood pressure and heart rate at rest, should be measured in adolescents on stimulants, as recommended by the American Academy of Child and Adolescent Psychiatry 1, 2
  • A cardiovascular history should be obtained to screen for symptomatic cardiovascular disease, family history of sudden cardiac death, structural heart defects, or arrhythmias, according to the American Academy of Child and Adolescent Psychiatry 1

Decision Making

  • Stimulants are contraindicated in symptomatic cardiovascular disease, as stated by the American Academy of Child and Adolescent Psychiatry 1, 2
  • If palpitations worsen, become frequent, or are associated with concerning symptoms, the medication should be held immediately and an ECG and cardiology consultation should be obtained, as recommended by the American Academy of Child and Adolescent Psychiatry 1

Optimization Strategies

  • Consistent daily dosing of stimulants, taken at the same time each morning after breakfast, can help minimize peak-related side effects, according to the American Academy of Child and Adolescent Psychiatry 2
  • A multimodal approach, combining medication with psychoeducation and behavioral interventions, can help maximize ADHD symptom control, as recommended by the American Academy of Child and Adolescent Psychiatry and the Pharmacology and Therapeutics society 1, 4

Critical Considerations

  • The American Academy of Child and Adolescent Psychiatry recommends that effective stimulant therapy should not be abruptly discontinued for mild, transient side effects without proper cardiovascular evaluation 1
  • The American Academy of Child and Adolescent Psychiatry also recommends that red flags such as chest pain, syncope, dyspnea, or exercise-induced symptoms require immediate cardiac workup and medication hold 1
