Blood Pressure Management in CKD Stage 5 Hypertensive Emergency

Initial Management

  • Reduce blood pressure by 10-20% in the first hour using IV antihypertensive agents, such as nicardipine, labetalol, or clevidipine, in patients with CKD stage 5 and hypertensive emergency 1
  • Avoid rapid or excessive blood pressure lowering as CKD stage 5 patients are particularly vulnerable to acute kidney injury from hypoperfusion 2
  • Target mean arterial pressure reduction of approximately 10-15% initially, not immediate normalization to chronic targets, in patients with CKD stage 5 and hypertensive emergency 2

Chronic Blood Pressure Targets

  • The American College of Cardiology recommends a blood pressure goal of <130/80 mmHg for adults with CKD and hypertension, including those with CKD stage 5, in the chronic outpatient setting 3
  • The European Society of Cardiology recommends systolic blood pressure of 130-139 mmHg for CKD patients, including those with CKD stage 5, in the chronic outpatient setting 6
  • NICE guidelines recommend <140/90 mmHg for CKD patients, including those with CKD stage 5, in the chronic outpatient setting 6

Medication Selection

  • ACE inhibitors or ARBs should be first-line therapy once transitioned to oral agents in patients with CKD stage 5 and hypertension 2
  • Multiple antihypertensive agents are typically required to achieve target blood pressure in CKD stage 5 patients with hypertension 2

Monitoring Strategy

  • Continuous blood pressure monitoring is essential during IV antihypertensive therapy in patients with CKD stage 5 and hypertensive emergency 1
  • Monitor for signs of hypoperfusion, including worsening mental status, chest pain, or further decline in urine output, in patients with CKD stage 5 and hypertensive emergency 3
  • Check serum creatinine and potassium within 2-4 weeks of initiation of ACE inhibitors or ARBs in patients with CKD stage 5 and hypertension 1

Key Pitfalls to Avoid

  • Never aggressively lower blood pressure to chronic targets during the acute emergency phase, as this risks catastrophic hypoperfusion in patients with impaired autoregulation 3
  • Do not apply the KDIGO <120 mmHg target to CKD stage 5 patients, as they were excluded from the supporting evidence and face increased risks of adverse events, including acute kidney injury, falls, and fractures 6
  • Avoid excessive diastolic blood pressure lowering (<70 mmHg), which increases cardiovascular risk in CKD patients, including those with CKD stage 5 4
