Management of Urinary Tract Dysfunction in ARPKD with Congenital Hepatic Fibrosis

Diagnosis and Management of Urinary Tract Infections

  • The American Academy of Pediatrics recommends applying standard pediatric UTI treatment protocols for patients with ARPKD, as they do not require specialized infection management beyond what is used for otherwise healthy children 1, 2
  • Obtain urine and blood cultures when upper UTI is suspected, and perform renal ultrasonography to evaluate for complications 1, 2
  • There is no evidence to suggest an increased risk of complicated or prolonged infections in polycystic kidney disease patients 1, 2

Management of Hematuria

  • The American College of Nephrology recommends ultrasonography as first-line imaging to rule out stones or urinary tract obstruction in patients with macroscopic hematuria, which occurs in 5-15% of children with polycystic kidney disease 1, 2, 5
  • Gross hematuria before age 30-35 years is associated with worse renal survival in adults, making early identification and management critical for long-term outcomes 1, 2

Blood Pressure Management

  • The American Heart Association recommends targeting blood pressure control aggressively using ACE inhibitors or ARBs as first-line agents, with a goal BP of 110/75 mmHg for patients aged 18-49 years with early CKD if tolerated 3

Long-term Considerations

  • The National Kidney Foundation recommends kidney transplantation (preferably preemptive living-donor) as the preferred treatment for kidney failure, with some patients requiring combined liver-kidney transplantation 3
