Management of Glycosuria
Diagnostic Approach
- Glycosuria occurs when blood glucose levels exceed the renal threshold, causing glucose to appear in urine, typically around 10 mmol/L or 180 mg/dL 1, 3
- Initial evaluation should include measurement of blood glucose levels to differentiate between diabetic and non-diabetic causes, and HbA1c testing to assess glycemic control over the previous 2-3 months 2
- Assessment of renal function through eGFR and urinalysis is necessary to identify potential renal causes 4
Management Algorithm
- If blood glucose is elevated with glycosuria, proceed with diabetes evaluation, and consider metformin as first-line therapy unless contraindicated 1, 3, 5
- For patients with severe hyperglycemia, consider immediate insulin therapy, which may be tapered once symptoms resolve, and combination therapy may be needed from the outset 5
- Lifestyle modifications, including weight reduction, regular physical activity, and dietary modifications to reduce carbohydrate intake, are essential for all patients 5, 6
Glycemic Targets
- Target HbA1c <7.0% for most patients to reduce microvascular complications, with more stringent targets for selected patients and less stringent targets for patients with history of severe hypoglycemia or extensive comorbidities 2, 7
