Propafenone Therapy Risks and Precautions

Serious Cardiovascular Side Effects

  • The American College of Cardiology notes that propafenone can cause serious cardiovascular side effects, including ventricular tachycardia and ventricular fibrillation, with a reported incidence of 1-5% depending on dose 4
  • QRS prolongation, an increase >25% from baseline, indicates potential proarrhythmic risk, as reported by the American Heart Association 2
  • Rapid atrial flutter with 1:1 AV conduction is a potential side effect, with the American College of Cardiology recommending careful monitoring 5
  • Worsening of heart failure is a possible side effect, with the American Heart Association noting that patients with heart failure or reduced left ventricular ejection fraction should avoid propafenone 2

Contraindications and Special Precautions

  • The American Heart Association recommends that propafenone be avoided in patients with heart failure or reduced left ventricular ejection fraction, as well as those with structural heart disease 2
  • Patients with severe obstructive lung disease, sinus or AV conduction disease, cardiogenic shock, hypotension, or Brugada syndrome should also avoid propafenone, according to the American College of Cardiology 5

Monitoring Recommendations

  • Regular ECG monitoring is essential during initiation and dose adjustments of propafenone therapy, with the American College of Cardiology recommending careful monitoring for QRS prolongation 5
