Pediatric Salbutamol Administration Guidelines

Dosage and Administration

  • Regular use of salbutamol syrup exceeding twice weekly for symptom control indicates poor control and need for controller medication adjustment, as recommended by the American Academy of Pediatrics 1

Alternative Delivery Methods

  • For acute management, nebulized salbutamol is often preferred with the following dosing:
  • Metered-dose inhaler: 4-8 puffs (90 μg per puff) every 15-20 minutes for 3 doses, then every 1-4 hours as needed, according to the American Academy of Pediatrics 5

Special Situations

  • For children with upper respiratory infections under age 6 requiring anesthesia, nebulized salbutamol (2.5 mg for <20 kg, 5 mg for >20 kg) administered 30 minutes before induction can reduce perioperative respiratory complications by approximately 50%, as recommended by the American Society of Anesthesiologists 2, 3, 4, 6

Comparison with Levalbuterol

  • Levalbuterol is the R-isomer of racemic albuterol and is effective at half the dose of racemic albuterol, as stated by the American Academy of Pediatrics 5, 1
  • When using levalbuterol instead of racemic albuterol, the dose should be halved, according to the American Academy of Pediatrics 1
