Administering As-Needed Labetalol to Patients on Oral Metoprolol
Pharmacological Considerations
- The European Society of Cardiology recommends that administering labetalol as needed to patients already taking oral metoprolol is not recommended due to the risk of excessive beta-blockade that could lead to bradycardia, hypotension, or cardiogenic shock 1, 2
- Concurrent use of metoprolol and labetalol creates a risk of additive beta-blockade effects, potentially causing bradycardia 1, hypotension 3, heart failure exacerbation 4, and cardiogenic shock in susceptible patients 1, 5
- Metoprolol is a selective beta-1 blocker 2
Risk Assessment
- Patients at highest risk for complications from excessive beta-blockade include those with age >70 years 5, 4, systolic blood pressure <120 mmHg 5, 4, heart rate <60 bpm or >110 bpm 5, 4, signs of heart failure or low cardiac output 6, Killip class >1 (presence of heart failure) 5, PR interval >0.24 seconds 6, second or third-degree heart block 6, and active asthma or reactive airway disease 6
Alternative Approaches
- The American College of Cardiology recommends considering non-beta-blocker alternatives for acute blood pressure management, such as calcium channel blockers (e.g., nicardipine) 1, ACE inhibitors or ARBs for long-term control 3, 6, and nitrates for hypertension with ongoing chest pain 6
- In specific clinical scenarios where both agents might be considered, such as acute aortic dissection, combining different beta-blockers is not recommended, and a single agent with adequate dosing should be used 1
- For severe pre-eclampsia, labetalol is recommended, but should replace rather than supplement metoprolol 1
Special Considerations
- When transitioning between beta-blockers, the American Heart Association recommends gradually tapering the first beta-blocker before initiating the second to avoid excessive beta-blockade 2
- During any transition period, vital signs should be monitored closely 7
- In the rare circumstance where labetalol might be considered in a patient on metoprolol, continuous cardiac monitoring should be provided 7, resuscitation equipment should be immediately available 7, and preparation should be made to treat bradycardia or hypotension 7
