Enoxaparin Dosing and Duration for NSTEMI
Standard Dosing Regimen
- The American College of Cardiology recommends administering enoxaparin 1 mg/kg subcutaneously every 12 hours for NSTEMI patients, continued for the duration of hospitalization or until PCI is performed 1, 2, 3, 4
- An initial 30 mg intravenous bolus of enoxaparin has been used in selected NSTEMI patients, though this is not universally required 2, 3, 4
- The standard duration of enoxaparin therapy for NSTEMI patients is throughout hospitalization or until PCI is performed 1, 2, 3, 4
Critical Dose Adjustments
- For NSTEMI patients with creatinine clearance <30 mL/min, the American College of Cardiology recommends reducing the enoxaparin dose to 1 mg/kg subcutaneously once daily 1, 2, 3, 4
- Calculating creatinine clearance is crucial in all NSTEMI patients before initiating enoxaparin, as renal impairment is common in older patients and significantly affects dosing 2, 3, 4
Management at Time of PCI
- If PCI occurs within 8 hours of the last subcutaneous enoxaparin dose, generally adequate anticoagulation exists, and no additional enoxaparin is needed 5
- If PCI occurs 8-12 hours after the last enoxaparin dose, an additional 0.3 mg/kg IV enoxaparin should be administered at the time of PCI 5
- If PCI occurs >12 hours after the last enoxaparin dose, patients should be treated as de novo anticoagulation with a full-dose regimen 5
Critical Safety Considerations
- The American College of Cardiology advises against administering UFH to NSTEMI patients already on enoxaparin during PCI, as this "stacking" significantly increases bleeding risk 5
- The SYNERGY trial demonstrated increased bleeding complications when patients on upstream enoxaparin received additional UFH at PCI 2, 3, 5
- Enoxaparin has superior efficacy compared to UFH in reducing recurrent ischemic events in NSTEMI patients 2, 3, 4
