Examination Techniques for Splenomegaly

Patient Positioning and Examination Approach

  • Position the patient supine or in right lateral decubitus position to optimize access to the left upper quadrant, as recommended by guidelines from medical societies, with the goal of improving diagnostic accuracy for detecting splenomegaly 1
  • A grading system based on palpable distance below left costal margin (LCM) can be used to assess splenomegaly, with scores ranging from 0.5 (normal spleen) to 4 (spleen extends to the right rib cage), providing a standardized approach to evaluation 1

Grading and Assessment of Splenomegaly

  • The grading system for splenomegaly includes:
  • Accurate measurement of spleen size is necessary for monitoring disease progression, and additional imaging may be considered when physical examination is equivocal or in obese patients where palpation and percussion are less reliable 1

Imaging Confirmation of Splenomegaly

Gold‑Standard Imaging Modality

  • In patients with suspected splenomegaly or equivocal physical‑exam findings, contrast‑enhanced computed tomography (CT) is regarded as the gold‑standard technique for detailed characterization of splenic abnormalities, providing superior diagnostic accuracy compared with other imaging methods. 2
