Management of Urinalysis with Leukocytes but Negative Nitrite
Diagnostic Interpretation
- A urinalysis showing leukocytes but negative nitrite is not sufficient to diagnose a urinary tract infection (UTI) without accompanying symptoms, as stated by the Infectious Diseases Society of America 1, 3
- The absence of pyuria has excellent negative predictive value and can effectively rule out UTI, according to the American Geriatrics Society 2, 4
- Nitrite testing has high specificity but low sensitivity for UTI detection, as reported by the JAMA network open 5
- Leukocyte esterase has higher sensitivity but lower specificity for UTI detection, as reported by the JAMA network open 5
Management Algorithm
- The American College of Physicians recommends treating only symptomatic patients with leukocytes but negative nitrite on urinalysis, with symptoms such as acute onset of dysuria, frequency, urgency, or gross hematuria 1, 2
- The Infectious Diseases Society of America suggests obtaining urine culture before starting antibiotics in symptomatic patients with positive leukocyte esterase, even with negative nitrite 1, 3
- The American Geriatrics Society recommends not performing urine culture in asymptomatic patients, regardless of leukocyte status 7, 2
Special Populations
- The American Geriatrics Society advises avoiding treatment based on non-specific symptoms without specific urinary symptoms in elderly patients, and asymptomatic bacteriuria is common in long-term care facility residents and does not require treatment 7, 2, 4, 8
- The European Urology association notes that the absence of both leukocyte esterase and nitrite on dipstick effectively excludes UTI even in elderly patients 2, 6
Common Pitfalls to Avoid
- The American Geriatrics Society warns that treating asymptomatic bacteriuria leads to unnecessary antibiotic use and resistance, and relying solely on dipstick results without considering clinical symptoms leads to overtreatment 7, 5, 2
- The European Urology association cautions against treating non-specific symptoms in elderly patients as UTI without clear urinary symptoms 7, 6
