Hypertension Management

Initial Treatment

  • A thiazide-type diuretic should be used as the first-line medication for most patients with uncomplicated hypertension, either alone or in combination with other agents, as recommended by the European Heart Journal and Hypertension guidelines 1, 2, 3, 4
  • The American Heart Association recommends starting with a single agent, such as a thiazide diuretic, and titrating the dose as needed after 2-4 weeks, with a strength of evidence based on extensive clinical trial data 2, 3
  • For patients with specific comorbidities, such as heart failure, the European Heart Journal recommends an ACE inhibitor (e.g., lisinopril) + beta-blocker + diuretic, with a high strength of evidence based on clinical trials 1
  • For black patients, the Hypertension guideline recommends a thiazide diuretic or calcium channel blocker (CCB), with a moderate strength of evidence based on clinical trials 2, 5

Combination Therapy

  • The European Heart Journal and Hypertension guidelines recommend starting with a two-drug combination, such as a thiazide diuretic + ACE inhibitor, ARB, beta-blocker, or CCB, for patients who require multiple medications to achieve target blood pressure, with a high strength of evidence based on clinical trials 2, 4
  • If blood pressure remains uncontrolled on a thiazide diuretic alone, the European Heart Journal and Hypertension guidelines recommend adding a second agent from a different class (ACE inhibitor, ARB, CCB, or beta-blocker) based on the patient's comorbidities and risk factors, with a moderate strength of evidence based on clinical trials 1, 2, 6

Special Considerations

  • The European Heart Journal recommends considering comorbidities, such as chronic kidney disease or diabetes, when selecting a first-line agent, with a high strength of evidence based on clinical trials 1
  • The Hypertension guideline recommends considering racial differences in response to antihypertensive medications, with black patients responding better to thiazide diuretics or CCBs, with a moderate strength of evidence based on clinical trials 2, 5
  • The American Heart Association recommends monitoring patients every 2-4 weeks until blood pressure is controlled, with a high strength of evidence based on clinical trials 6

Rationale for Thiazide Diuretics

  • Thiazide diuretics are recommended as first-line therapy due to their proven efficacy in reducing cardiovascular events and mortality, low cost, and favorable safety profile at low doses, with a high strength of evidence based on extensive clinical trial data 3, 4, 6
  • The largest head-to-head comparison of first-step antihypertensive medications found that the thiazide diuretic chlorthalidone was superior to the CCB amlodipine and the ACE inhibitor lisinopril in preventing heart failure, with a high strength of evidence based on clinical trials 4
