Perioperative Management of Pentoxifylline for Orthopedic Surgery

General Recommendations

  • The American College of Chest Physicians recommends that pentoxifylline should be discontinued 2-3 days before orthopedic surgery due to its reversible antiplatelet properties and half-life of 2-10 hours 1
  • Pentoxifylline has reversible antiplatelet effects with a half-life of 2-10 hours, allowing for a shorter discontinuation period compared to irreversible antiplatelet agents 1

Patient-Specific Considerations

  • For patients with renal impairment, the discontinuation period of pentoxifylline may need to be extended to 3 days due to potential drug accumulation 2
  • For elderly patients (>80 years), the standard 2-3 day discontinuation period of pentoxifylline is recommended 1
  • For patients on multiple antithrombotic medications, the discontinuation period should be determined by the agent requiring the longest interruption 3, 4

Resumption After Surgery

  • Pentoxifylline can typically be resumed 12-24 hours after surgery when adequate hemostasis has been achieved, but may be delayed for 48-72 hours after high bleeding risk orthopedic procedures 4

Comparison with Other Agents

  • The discontinuation period for pentoxifylline (2-3 days) is shorter than that for aspirin/clopidogrel (7-10 days) and warfarin (5 days), but similar to that for direct oral anticoagulants (2-5 days) 1, 2, 4
