Medical Necessity of Endovenous Ablation Therapy for Varicose Veins
Assessment of Medical Necessity Criteria
- The patient presents with bilateral lower extremity symptoms including restless legs, aching, and cramping associated with documented varicose veins 2, 3
- The American Academy of Family Physicians notes that varicose veins can cause varying degrees of discomfort including pain, aching, heaviness, cramping, throbbing, and restlessness in the legs 4, 5
Unmet Criteria for Medical Necessity
- Current guidelines require incompetence at the saphenofemoral junction or saphenopopliteal junction to be documented by recent ultrasound performed within the past 6 months 1
- The American College of Phlebology recommends documentation of a 3-month trial of medical grade (20 mmHg or greater) gradient support compression stockings 1
Treatment Algorithm for Varicose Veins
- The American Academy of Family Physicians recommends conservative management with medical grade compression stockings (20 mmHg or greater) for at least 3 months 1, 6
- Endovenous thermal ablation (radiofrequency or laser) is recommended as first-line interventional treatment for GSV or SSV with documented reflux 1
