Domperidone Dosage for Gastroesophageal Reflux Disease in Adults
Introduction to Domperidone Therapy
- The American Gastroenterological Association recommends starting with a dose of 10 mg three times a day as the initial dose, with the possibility of increasing to 20 mg three to four times a day if necessary, while avoiding exceeding 30 mg/day due to the risk of QTc prolongation and sudden cardiac death 1
Dosage Regimen
- The initial dose of 10 mg three times a day minimizes the cardiovascular risk while providing prokinetic and antiemetic effects 1
- The maximum dose should not exceed 20 mg three to four times a day, with a total daily dose of 60-80 mg, if the response is inadequate 1
Safety Considerations
- The European Medicines Agency recommends against the long-term use of prokinetic agents like domperidone due to cumulative cardiac risks 2
- Patients over 60 years have a higher risk of cardiac complications 1
- Domperidone is contraindicated in patients with pre-existing QTc prolongation, electrolyte abnormalities (hypokalemia, hypomagnesemia), and concomitant use of QT-prolonging medications 2
Alternative Therapies
- When domperidone is contraindicated, alternative therapies include 5-HT3 receptor antagonists like ondansetron 4-8 mg two to three times a day 1, 2
- Metoclopramide 5-20 mg three to four times a day can be used for up to 12 weeks due to the risk of tardive dyskinesia 1, 3
- Prucalopride is a first-line prokinetic agent without cardiac effects 3
