Early Invasive Strategy for High‑Risk Non‑ST‑Elevation Acute Coronary Syndrome
Risk Stratification
- Patients with elevated cardiac troponin are classified as having NSTEMI and meet high‑risk criteria that mandate early coronary angiography during the index hospitalization. European Society of Cardiology (ESC) guideline – Class I, Level A. 1
- Diabetes mellitus is identified by ESC as an independent high‑risk feature that requires an early invasive management approach in NSTE‑ACS. ESC guideline – Class I, Level A. 5
- The presence of multiple cardiovascular risk factors (e.g., hypertension, hyperlipidaemia, medication non‑adherence) further categorises patients as high‑risk, supporting an early invasive strategy. ESC guideline – Class I, Level B. 2
- Uncontrolled hypertension on presentation (e.g., systolic ≥ 160 mm Hg) is another high‑risk marker that ESC recommends be addressed with prompt angiography. ESC guideline – Class I, Level B. 3
Diagnostic Timing
- Once NSTEMI is confirmed by troponin elevation, additional serial troponin measurements do not change management and only delay definitive therapy. ESC guideline – Class IIa, Level B. 2
- BNP/NT‑proBNP testing is not required for immediate decision‑making in patients who already meet high‑risk criteria; ordering it would unnecessarily postpone angiography. ESC guideline – Class IIb, Level C. 1
- Stress testing (including adenosine nuclear stress) is contraindicated in the acute phase of NSTEMI and is reserved for low‑risk patients with normal biomarkers after 8–12 h of observation. ESC guideline – Class III, Level A. 4
Pharmacologic Management Prior to Angiography
- Therapeutic unfractionated heparin should be continued in all high‑risk NSTE‑ACS patients awaiting angiography. ESC guideline – Class I, Level A. 5
- GP IIb/IIIa receptor blockers (tirofiban or eptifibatide for 24 h; abciximab for 12 h if PCI is performed) are recommended as adjunctive therapy in high‑risk patients. ESC guideline – Class IIa, Level B. 4
- Aspirin, β‑blocker, and nitrates should be maintained throughout the pre‑procedural period. ESC guideline – Class I, Level A. 3
- Clopidogrel should be withheld when coronary artery bypass grafting is anticipated within the next 5 days, to reduce peri‑operative bleeding risk. ESC guideline – Class IIb, Level C. 1
Timing of Coronary Angiography
- Coronary angiography must be performed during the index hospitalization and as soon as logistically feasible for patients with elevated troponin and diabetes. ESC guideline – Class I, Level A. 5
- Immediate angiography within the first hour is reserved for patients with hemodynamic instability, major arrhythmias, or ongoing severe ischemia; otherwise, the procedure should not be delayed beyond the current admission. ESC guideline – Class IIa, Level B. 2
Critical Pitfalls to Avoid
- Do not discharge high‑risk NSTEMI patients for outpatient stress testing; inpatient angiography is required when troponin is elevated and diabetes is present. ESC guideline – Class I, Level A. 4
- Do not postpone angiography for additional biomarker measurements once NSTEMI is established and high‑risk features are present. ESC guideline – Class I, Level A. 5
