Diagnostic Approach to Elevated Blood Cell Counts
Introduction to Myeloproliferative Neoplasms
- The combination of elevated RBC parameters with thrombocytosis, lymphocytosis, and eosinophilia suggests a myeloproliferative neoplasm with tyrosine kinase gene fusion, particularly BCR::ABL1-positive chronic myeloid leukemia (CML), according to the National Comprehensive Cancer Network 1, 2
- Myeloid/lymphoid neoplasms with PDGFRA, PDGFRB, FGFR1, JAK2, ABL1, or FLT3 rearrangements must be excluded because they respond to tyrosine kinase inhibitors, as recommended by the Journal of the National Comprehensive Cancer Network 3, 1, 2
Diagnostic Evaluation
- A complete blood count with manual differential is essential to determine absolute eosinophil count, basophil percentage, blast percentage, and promyelocyte count, as stated by Leukemia 8, 1, 2
- Serum tryptase and vitamin B12 levels are elevated in myeloproliferative variants and provide crucial diagnostic information, according to the Journal of the National Comprehensive Cancer Network 3, 4, 2
- Bone marrow aspirate and biopsy with morphology assessment for hypercellularity, blast percentage, and focal "nests of blasts" are mandatory for definitive diagnosis, as recommended by the Journal of the National Comprehensive Cancer Network and Leukemia 3, 8, 1
Exclusion of Secondary Causes
- Strongyloides serology is mandatory, especially with travel history, to exclude parasitic infections, as stated by the Journal of the National Comprehensive Cancer Network and Praxis Medical Insights 3, 2
- Allergic disorders account for 80% of eosinophilia cases but cause only mild elevations and would not explain the pancytosis, according to Praxis Medical Insights 1
- Medication-induced eosinophilia should be considered, but the combination with elevated RBC parameters and thrombocytosis makes this less likely, as noted by the Journal of the National Comprehensive Cancer Network and Praxis Medical Insights 3, 7
Diagnostic Algorithm
- Confirm absolute values for all cell lines and calculate absolute eosinophil count, as recommended by Leukemia and Praxis Medical Insights 8, 1
- Measure serum tryptase, vitamin B12, and EPO levels immediately, according to the Journal of the National Comprehensive Cancer Network, Mayo Clinic Proceedings, and Praxis Medical Insights 3, 4, 5, 2
- Proceed directly to bone marrow evaluation with comprehensive cytogenetic and molecular testing for tyrosine kinase fusions, as stated by the Journal of the National Comprehensive Cancer Network and Praxis Medical Insights 3, 1, 2
