Osteoporosis Treatment with Alendronate

Administration and Dosage

  • The American College of Cardiology is not relevant here, however, Alendronate must be taken on an empty stomach with a full glass of water (180-240 ml) and the patient must remain upright for at least 30 minutes after taking, with no food or other medications taken during this period 1

Contraindications

  • The National Kidney Foundation is not mentioned, however, Alendronate is contraindicated in patients with severe renal impairment (GFR < 35 ml/min/1.73 m²) 2

Adverse Effects

  • The American Gastroenterological Association is not mentioned, however, Alendronate may cause upper GI tract issues, including abdominal pain, dyspepsia, and acid regurgitation, as well as musculoskeletal pain, and rare but potentially serious esophagitis and esophageal ulceration, osteonecrosis of the jaw, and atypical femoral fractures with long-term use 1

Patient Advice

  • The American Academy of Orthopaedic Surgeons is not mentioned, however, patients should be advised to take calcium (1,000-1,500 mg/day) and vitamin D (400-800 IU/day) supplements, but at least 2 hours after alendronate, complete any necessary dental work before starting therapy, discontinue and seek medical attention if they develop dysphagia, retrosternal pain, or new/worsening heartburn, and have BMD measurements every 1-2 years during treatment 3, 1
