Baclofen Monitoring and Management

Severe Withdrawal Syndrome

  • Abrupt discontinuation of baclofen can lead to potentially life-threatening withdrawal symptoms, especially in patients on long-term therapy, according to the Mayo Clinic Proceedings 1, 2, 3
  • Withdrawal symptoms include visual and auditory hallucinations, anxiety, agitation, delirium, fever, tremors, tachycardia, and seizures, as reported by the Mayo Clinic Proceedings 1, 3
  • Intrathecal baclofen withdrawal is particularly dangerous and can cause high fever, altered mental status, rebound spasticity, muscle rigidity, rhabdomyolysis, multiorgan failure, and death, as stated by the Mayo Clinic Proceedings 1, 3

Special Populations and Considerations

  • Elderly patients are at higher risk for adverse effects and typically cannot tolerate doses greater than 30-40 mg per day, as recommended by Praxis Medical Insights 4
  • Starting dose should be lower in elderly patients (5 mg three times daily) to minimize adverse effects, according to Praxis Medical Insights 4
  • Drug interactions, such as increased sedation, can occur when baclofen is used with opiates or other CNS depressants, as reported by the Mayo Clinic Proceedings 1, 2

Monitoring and Management

  • Dose adjustments should consider gradual dose increases to minimize side effects, as recommended by Praxis Medical Insights 4
  • Discontinuation of baclofen should always be tapered gradually to avoid withdrawal syndrome, according to the Mayo Clinic Proceedings 1, 3
  • For intrathecal baclofen, perioperative management should involve specialist input, as stated by the Mayo Clinic Proceedings 1, 3
  • Timing adjustments, such as administering medication with meals, may reduce gastrointestinal side effects, as suggested by Praxis Medical Insights 5
