Pediatric Dosing for Topical Lidocaine
Age-Based Dosing Guidelines
- The American Academy of Pediatrics recommends that lower concentration topical lidocaine preparations (4% or less) are routinely used in pediatric emergency and procedural settings under medical supervision 1
Context: When Topical Lidocaine IS Used in Pediatrics
- Topical 4% liposomal lidocaine (LMX4) provides anesthesia in approximately 30 minutes and is commonly used for IV line placement or venipuncture, lumbar puncture, abscess drainage, and joint aspiration under medical supervision 1
- Critical age-specific consideration: The topical anesthetic dose should be lower for patients <12 months old or weighing <10 kg 1
Maximum Systemic Doses When Infiltrative Lidocaine Is Used
- For infiltrative (injected) lidocaine in children: without epinephrine, the maximum dose is 1.5-2.0 mg/kg, and with epinephrine, the maximum dose is 3.0-4.5 mg/kg 2
- For infants under 6 months: all amide local anesthetic doses should be reduced by 30% 3
Critical Safety Warnings
- Contraindications for topical lidocaine use include allergy to amide anesthetics, non-intact skin, and recent sulfonamide antibiotic use 1
- Signs of lidocaine toxicity to monitor include circumoral numbness, facial tingling, metallic taste, and auditory changes 4
- Severe manifestations of lidocaine toxicity include seizures, CNS depression, and cardiac arrhythmias 3, 4, 5
