Management of Hepatic Encephalopathy

Introduction to Hepatic Encephalopathy Management

  • The American Association for the Study of Liver Diseases recommends that all patients with hepatic encephalopathy should receive immediate treatment with lactulose as first-line therapy while simultaneously identifying and correcting precipitating factors, which alone resolves approximately 90% of cases 1, 2
  • The management follows a structured four-pronged approach, including initiating basic care, excluding alternative causes of altered mental status, identifying and correcting precipitating factors, and beginning empirical lactulose therapy 1

Patient Assessment and Triage

  • Patients with grades 0-2 hepatic encephalopathy should be managed on a general medicine ward with frequent mental status monitoring, and transferred to ICU if consciousness deteriorates 3
  • Immediate ICU admission is mandatory for patients with grades 3-4 hepatic encephalopathy who are unable to protect their airway, as they are at high risk for aspiration and require intensive monitoring 2

Pharmacological Treatment

  • Lactulose 25 mL orally every 12 hours, titrated to achieve 2-3 soft bowel movements per day, is the recommended first-line treatment for hepatic encephalopathy, and can be administered via nasogastric tube in patients unable to swallow or at aspiration risk 2, 1
  • Rifaximin 550 mg orally twice daily should be added to lactulose for secondary prophylaxis after more than one additional episode of overt hepatic encephalopathy within 6 months, as it reduces recurrence risk by 58% 2

Secondary Prophylaxis and Liver Transplantation

  • After the first episode of overt hepatic encephalopathy, lactulose should be continued indefinitely as secondary prophylaxis, and rifaximin should be added after recurrent episodes despite lactulose therapy 2, 5
  • The first episode of overt hepatic encephalopathy should prompt referral to a transplant center for evaluation, as liver transplantation represents the definitive treatment for hepatic encephalopathy 2, 3

Diagnostic Considerations and Medications to Avoid

  • Plasma ammonia levels should be measured in suspected cases of hepatic encephalopathy, but should not be relied upon exclusively for diagnosis or monitoring 5, 4
  • Proton pump inhibitors, benzodiazepines, and all sedatives should be avoided in patients with hepatic encephalopathy due to increased risk of worsening encephalopathy or other adverse effects 4, 3
