Antibiotic Use in Coronary Artery Disease Patients

Safe Antibiotic Options

  • The American Heart Association recommends continuing all guideline-directed CAD therapies, including aspirin, P2Y12 inhibitors, beta-blockers, ACE inhibitors/ARBs, and statins, during antibiotic treatment 1, 2, 3
  • Cefixime is noted as a safe alternative in CAD patients, with no interference with essential cardiovascular medications, and patients should continue their primary CAD medications without interruption while taking cefixime 1
  • Dose adjustment for cefixime is only needed when creatinine clearance falls below 20 mL/min/1.73 m² 1

General Treatment Considerations

  • The European Society of Cardiology suggests prioritizing penicillins, cephalosporins, or tetracyclines over macrolides and quinolones when treating infections in CAD patients 1
  • If clarithromycin must be used, treatment duration should be limited and the indication should be compelling, due to the FDA warning regarding increased long-term mortality risk in CAD patients 1
