Treatment of Renal Tuberculosis
Standard Treatment Regimen
- The American Thoracic Society recommends a 6-month regimen for renal tuberculosis, consisting of 2 months of isoniazid, rifampin, pyrazinamide, and ethambutol, followed by 4 months of isoniazid and rifampin, with critical dose adjustments for renal function 1, 2
- The standard treatment regimen for extrapulmonary tuberculosis, including renal TB, follows the same approach as pulmonary disease, with an initial intensive phase and a continuation phase 1, 2
- The total duration of treatment is 6 months for most cases of extrapulmonary TB 2
Critical Dose Adjustments for Renal Impairment
- Isoniazid, rifampin, and pyrazinamide are predominantly metabolized by the liver and can be given at standard doses in renal failure 3, 4
- Ethambutol requires significant dose reduction based on creatinine clearance, with recommended doses of 25 mg/kg body weight for creatinine clearance 50-100 mL/min, 25 mg/kg twice weekly for creatinine clearance 30-50 mL/min, and 15 mg/kg every 36-48 hours for creatinine clearance 10-30 mL/min 3, 4
- Serum drug levels should be monitored in renal impairment 3, 4
Special Monitoring Requirements
- Before initiating therapy in patients with renal disease, check renal function, test visual acuity, perform baseline audiogram and vestibular testing, and establish baseline liver function tests 8, 3, 4, 5, 6
- Monthly renal function assessment and questioning about visual symptoms or auditory/vestibular symptoms are recommended during treatment 8, 5, 6
- Serum drug concentration monitoring is recommended for ethambutol, cycloserine, or injectable agents in renal impairment 1
Medication Administration
- All anti-TB medications should be administered after hemodialysis sessions to prevent premature drug removal and maintain therapeutic levels between dialysis sessions 1, 7
- If streptomycin or other aminoglycosides are used, dosing frequency must be reduced to 2-3 times weekly in renal insufficiency, and the milligram dose should be maintained at 12-15 mg/kg per dose 1, 5, 6
- Monitor serum drug concentrations to avoid ototoxicity and nephrotoxicity 1, 6
