Estrogen Patch Treatment Regimen for Menopausal Symptoms

Risk-Benefit Considerations

  • Estrogen therapy is most effective for vasomotor symptoms, reducing hot flashes by approximately 75% 1
  • The benefit-risk profile is most favorable for women under 60 years of age or within 10 years of menopause onset 1
  • For every 10,000 women taking estrogen and progestin for 1 year, there may be 7 additional CHD events, 8 more strokes, 8 more pulmonary emboli, and 8 more invasive breast cancers, balanced against 6 fewer cases of colorectal cancer and 5 fewer hip fractures 1

Special Considerations

Absolute Contraindications

  • History of breast cancer or other hormone-sensitive cancers 1, 2
  • Active liver disease 1, 2
  • History of abnormal vaginal bleeding 2
  • History of venous thromboembolism or stroke 1
  • Antiphospholipid syndrome 1

Common Pitfalls to Avoid

  • Initiating hormone therapy solely for prevention of chronic conditions like osteoporosis or cardiovascular disease rather than for symptom management 3, 1
  • Using estrogen therapy without progestin in women with an intact uterus, which increases endometrial cancer risk 1
