Optimal Dosage of Vitamin B12 and Folic Acid in Patients with Hemodialysis
Folic Acid - Dosage and Indications
- The typical dose is 1-5 mg of folic acid daily by mouth to compensate for losses during dialysis, as recommended by the Clinical Nutrition guideline 1
- Patients on hemodialysis lose approximately 0.3 mg of folic acid daily in the dialysate, justifying the need for supplementation, according to Clinical Nutrition 3
- For non-diabetic patients with hyperhomocysteinemia, 5 mg or more of folic acid daily by mouth is recommended, as suggested by Clinical Nutrition 1
- For diabetic patients with hyperhomocysteinemia, 15 mg of folic acid daily by mouth is recommended, based on Clinical Nutrition guidelines 1
- The upper limit of safety is 1 mg/day to avoid delayed diagnosis of vitamin B12 deficiency, but higher doses are used under monitoring, as stated by Clinical Nutrition 1
Vitamin B12 - Dosage and Indications
- The American College of Clinical Nutrition recommends that vitamin B12 should not be administered routinely to all patients with hemodialysis, but only when deficiency is documented or for prevention of deficiency during long-term folic acid treatment 1
Monitoring and Evaluation
- Initial assessment should include measuring serum folate and red blood cell folate levels in patients with macrocytic anemia or risk of malnutrition, as recommended by Clinical Nutrition 1
- The normal levels are: serum folate ≥10 nmol/L and red blood cell folate ≥340 nmol/L, according to Clinical Nutrition 1
- Measurements should be repeated within 3 months after starting supplementation to confirm normalization, and then annually, as suggested by Clinical Nutrition 1
- In patients with increased needs, measurements should be taken every 3 months until stabilization, then annually, based on Clinical Nutrition guidelines 1
- Patients with good dietary intake or special renal formulas may not require additional supplements, as stated by Praxis Medical Insights 5
- The Clinical Nutrition guideline recommends that folic acid supplementation should not be started without checking B12 levels, as it may mask hematological deficiency while allowing neurological complications to progress 1
