Dexamethasone Use in Pediatric Patients

Approved Indications and Dosing

  • The American Academy of Pediatrics supports the use of dexamethasone for croup, with a single dose of 0.6 mg/kg (maximum 16 mg) being the standard treatment 1, 2
  • For a typical 1-year-old weighing 10 kg, the dose translates to 6 mg as a single dose, which can be administered orally, intramuscularly, or intravenously, with all routes being equally effective 1, 2
  • The American Society of Clinical Oncology recommends dexamethasone in combination with 5-HT3 receptor antagonists for pediatric patients as young as 6 months receiving moderate or high-emetic-risk chemotherapy 4, 5
  • For bacterial meningitis, the Infectious Diseases Society of America recommends 0.15 mg/kg every 6 hours for 2-4 days, which must be initiated 10-20 minutes prior to or concomitant with the first antimicrobial dose 6, 1
  • The American Thoracic Society conditionally recommends dexamethasone for children less than 1 year at high risk of post-extubation upper airway obstruction, to be given at least 6 hours before extubation 7

Critical Safety Considerations

  • Single-dose regimens, like 0.6 mg/kg for croup, do not cause clinically significant adrenal suppression and require no tapering, according to the American Academy of Pediatrics 2
  • Short-term use for acute conditions is well-tolerated, with the American Academy of Pediatrics supporting dosing from 0.5 to 9 mg/day depending on disease severity 1, 2
  • The American Academy of Pediatrics advises against using high-dose dexamethasone for extended periods in young children, as it has been associated with adverse neurodevelopmental outcomes 1, 3

Condition-Specific Algorithms

  • For respiratory distress in a 1-year-old with barky cough and stridor, the American Academy of Pediatrics recommends giving dexamethasone 0.6 mg/kg (max 16 mg) once 1
  • For suspected bacterial meningitis, the Infectious Diseases Society of America recommends giving dexamethasone 0.15 mg/kg immediately, 10-20 minutes before antibiotics, and continuing every 6 hours for 2-4 days 6
