Meningococcal Vaccination Guidelines for Asplenic Patients

Key Differences in Vaccination Protocols

  • The American College of Physicians recommends that asplenic patients receive 2 doses of MenACWY vaccine at least 8 weeks apart, plus additional MenB vaccination, and mandatory revaccination every 5 years for life, due to increased infection risk 1, 2
  • For young adults, a single dose of MenACWY is recommended for routine vaccination, with no routine revaccination unless a high-risk condition develops 1, 2
  • MenB vaccination is mandatory for asplenic patients, but permissive, not mandatory, for young adults aged 16-23 years 1, 2

Timing Considerations

  • The World Health Organization suggests that optimal vaccination timing for splenectomy patients is at least 2 weeks before elective splenectomy, or at least 2 weeks post-operatively if emergency splenectomy 3

Critical Clinical Pitfalls

  • The Centers for Disease Control and Prevention advises against treating asplenic patients like routine young adults, and recommends the enhanced 2-dose MenACWY series, not a single dose, as well as mandatory MenB vaccination 1, 2
  • Lifelong revaccination with MenACWY every 5 years is essential, as protection wanes and infection risk persists for life 1, 2

Rationale for Enhanced Protocol

  • The World Health Organization notes that overwhelming post-splenectomy infection (OPSI) carries a high mortality rate of 30-70%, and that Neisseria meningitidis is one of the most common causative organisms 3
  • Asplenic patients have an impaired immune response, requiring both enhanced initial dosing and regular boosters 1, 2

Vaccination Recommendations for Asplenic Patients

Haemophilus influenzae type b (Hib) Vaccination

  • The National Comprehensive Cancer Network recommends that Hib vaccine should be administered at least 2 weeks before elective splenectomy for optimal immune response 4
  • For adults (≥15 months) undergoing elective splenectomy who are unimmunized, the Centers for Disease Control and Prevention recommends 1 dose prior to procedure 5
  • For asplenic patients aged >59 months and adults who are unimmunized, the Centers for Disease Control and Prevention recommends 1 single dose 5

Meningococcal Vaccination

  • The Centers for Disease Control and Prevention recommends primary vaccination for asplenic patients aged ≥10 years: 2 doses of MenACWY given ≥8 weeks apart 6, 7, 8
  • The Centers for Disease Control and Prevention recommends booster doses of MenACWY: Single dose at 5 years after primary vaccination and every 5 years thereafter if the person remains at increased risk 6, 9
  • For asplenic patients aged ≥10 years, the Centers for Disease Control and Prevention recommends MenB-FHbp: 3-dose series at 0, 1-2, and 6 months, OR MenB-4C: 2-dose series given ≥1 month apart 6, 7, 9, 10
  • The Centers for Disease Control and Prevention recommends booster doses of MenB: Single dose at 1 year after completion of primary vaccination and every 2-3 years thereafter 6
