Optimal Treatment for Inflammatory Breast Carcinoma
Introduction to Treatment Guidelines
- The American College of Oncology recommends that patients with inflammatory breast cancer receive trimodality therapy consisting of neoadjuvant systemic chemotherapy, followed by modified radical mastectomy, and then postmastectomy radiation therapy 1, 2, 3
Systemic Chemotherapy
- All patients with inflammatory breast cancer should receive primary systemic chemotherapy before any surgical intervention, with a standard cytotoxic backbone of anthracycline-based therapy combined with taxanes 1, 3, 4
- For HER2-positive inflammatory breast cancer, trastuzumab should be added to the chemotherapy regimen 1, 3, 4
Surgical Management
- Modified radical mastectomy is the only acceptable definitive surgical approach following neoadjuvant chemotherapy, and breast-conserving surgery is not recommended for inflammatory breast cancer 1, 3, 4
- Immediate breast reconstruction is not recommended, but delayed reconstruction may be offered 3, 4
Radiation Therapy
- Postmastectomy radiation is mandatory for all inflammatory breast cancer patients, with a standard dose of comprehensive chest wall and regional nodal radiation 1, 3, 4
- An escalated dose to 66 Gy is recommended for high-risk patients, including those with age >45 years, close or positive surgical margins, or ≥4 positive lymph nodes after neoadjuvant therapy 3, 4
