Heart Failure Treatment with Furosemide and Lisinopril

Rationale for Combination Therapy

  • Diuretics, like furosemile, should always be administered in combination with ACE inhibitors, like lisinopril, in heart failure patients when fluid overload is present, as recommended by the European Society of Cardiology 1, 2
  • This combination addresses both symptom relief and disease-modifying treatment for improved outcomes in heart failure patients, according to the European Society of Cardiology 2, 3

Administration Guidelines

  • The European Society of Cardiology recommends starting with lisinopril 5 mg once daily for heart failure patients, and checking renal function and electrolytes before starting and 1-2 weeks after initiation 1, 5
  • For furosemide, the European Society of Cardiology suggests a starting dose of 20 mg, and adjusting the dose according to symptoms, signs of congestion, blood pressure, and renal function 3, 6

Important Precautions

  • The European Society of Cardiology advises avoiding potassium-sparing diuretics during initiation of ACE inhibitor therapy, and avoiding excessive diuresis before starting lisinopril 1, 5
  • Monitoring for hypotension, especially in patients with low baseline blood pressure, and checking renal function and electrolytes 1-2 weeks after initiation and after dose increases is recommended by the European Society of Cardiology 1, 5

Monitoring Recommendations

  • The European Society of Cardiology recommends checking blood pressure, renal function, and electrolytes 1-2 weeks after initiation, and monitoring for symptomatic hypotension, especially during the first few days 1, 3
  • Assessing for signs of improved fluid status, such as reduced edema and improved breathing, is also recommended by the European Society of Cardiology 2, 3

Potential Adjustments

  • If renal function deteriorates substantially, the European Society of Cardiology suggests considering stopping or reducing the dose of lisinopril 1, 5
  • If hypotension occurs, reducing the diuretic dose if no signs of congestion remain, or if insufficient response to diuretic, increasing the dose or considering twice-daily administration is recommended by the European Society of Cardiology 1, 6
