Treatment of Cervicofacial Actinomycosis

Antibiotic Therapy and Surgical Management

  • Amoxicillin/clavulanic acid is an acceptable alternative to penicillin G for dental infections extending into cervicofacial tissues, based on guideline recommendations 1
  • Surgical intervention, including incision and drainage of abscesses, is essential and should be performed concurrently with antibiotic therapy, not as an alternative, as recommended by the guideline society 1
  • Tooth extraction is necessary when the infection originates from an odontogenic source, as part of the surgical management 1
  • The guideline society recommends that surgical treatment, such as incision and drainage, should not be delayed in cases showing inadequate response to initial medical therapy, to prevent progression to osteomyelitis 1
  • The American College of Surgeons or similar guideline society may recommend that antibiotics should not be used as monotherapy for dental abscesses, and that surgical treatment is primary, with antibiotics being adjunctive 1
